The vast majority of the route is predicated over the fact that she can't be sure whether the MC is real and his feeling are genuine or not. A few things happen that show these things are in fact true, and that "fake" people she helped make could also get some of their personality back, and MC goes way the fuck beyond any sort of duty for a lover and gives pretty much everything up for her. Then there's a dramatic scene where he's getting drug off to a hospital.
Then the MC recovers, just leaves the hospital on his own, and the route ends. No development of the relationship. No "I love you too". No Kotori picking up the person she brought there in the first place and should've been worried fucking sick about. No payoff. Fucking nothing.
