Because standards change over time and people view things as less unacceptable. There was a time when you couldn't say ANY fucks in a PG-13 movie. There was a time when you couldn't call someone a dick on network television. But the times, they are a-changin' and the younger generations are less prudish and hung up about "muh evil child-mind-warping bad words" so it becomes more okay to swear in things without jacking up the rating.

Because there are really no rules, just the ratings board's opinions about what is acceptable.

The MPAA doesn't have rules for rating.
If they have exact rules, they're a censorship organization and that's obviously not true.
So they have suggestions and guidelines.

Directors submit their movies and they get a rating with no specific details.

Pretty sure Forrest Gump has at least three plus nudity.

Would make sense in an older movie like Airplane! or The Right Stuff, but that was the 90's when they had pretty much settled down the rating rules.
Anyone have an explanation for this one?
