Difference is he's carrying the horn of Gilead, which he previously abandoned because he essentially stopped caring about anything but revenge. This heavily implies that his experience leading up to the tower in the first books stayed with him and he regained his lost humanity, and the Tower basically allowed him to start again, and get it right. The book ends there, and everybody got mad.
