Moral Orel is an example of something where the first two seasons (bar S2 finale) are really just an extended prologue. It's a widely held conviction that the third season is the best, but its because the first two seasons kind of lull the viewer with false expectations.

Penny Dreadful seems to be a show that is getting better. I know season 3 just started but I'm pretty hyped for it from what the premiere showed.
The first season I only was watching it for Eva Green, it felt unfocused at first. The second got much more interesting but still took awhile to get going. The third seems to be already exciting and will have some interesting things planned (I hope).

I can't think of any that I've seen, although I haven't seen some of the shows mentioned in this thread. I've only watched one season of Mad Men, so it's possible that one does.
I can think of many shows that come close, but even the best ones peak 2 or 3 seasons short of the series finale.
The Shield. Starts out pretty well and just gets better and better, ending in a great final season.

Maintaining quality after 3 seasons is really hard because all the best ideas where used already but the series still does good enough and makes tons of money so ending it is hardly an option.
Admittedly most times the quality drops slowly and not abruptly not to mention some times it's the viewer the one that fails to capture what the new direction of the series is all about.
Some of the best episodes of some shows take place on really crappy seasons making me wonder if the level has really dropped or if it was me not getting the point.
