Because the Emperor feared a healthy Vader as an apprentice. He could have even gotten him proper synthetic organs and limbs, but instead slapped his ass into an unwieldy suit of army which took away all traces of humanity from him. He saw an opportunity to keep Vader shackled, to continue as the most powerful one man army, but not able to overthrow the Emperor and took it.
Vader quickly realized this. It's why he made the effort to convince Luke to join him.

Half of it was probably to create a Dragon but the other part was to create an almost inhuman appearance that went to dehumanize him in order to have Vader follow Sheev because he knew he could never defeat him alone. Plus sheev essentially told him that he was getting replaced by luke when he told him to get luke to join them.
Because Sheev knew he could kill Anakin with one lightning shot if he tried to usurp his throne. Basically got the best henchman while making him vulnerable to Sheev's best move that isn't spinning.
