The close up of Jack's eye in the final shot also suggests that it's all flashing before his eyes as he's dying, not after he dies. There's even a bit in a season 2 episode where a girl who had a near death experience meets mr. Eko in the airport with a message from his dead brother. She says she met him while she was "between places"

Christian literally says in the last episode that the flash-sideways was a place that all of the losties together created because the most important events of their lives took place on the Island. Every character in the flash sideways died, but they didn't all die at the same time, some lived to be old, some died on the Island, etc. The characters themselves reflect these traits, for example Jin and Sun have a better relationship in the flash sideways that reflects how they healed it before they both died in the sub. Jack is a man of faith in the flash sideways, he gives hope to John even though John somewhat lost his faith (relating to how ben killed him and his feeling of betrayal).
Kate is also different, in the show she didn't want to have a child and was afraid of commitment, which is why she was always on the fun, but she actually convinces claire to keep her baby in the flash sideways.
