Their proportions to each other in the flashback are the same as they've always been.
The problem is that we've lost the usual measuring stick: Steven. Instead we have Greg, who had a MUCH bigger head when he was younger but was actually just a bit shorter than modern Greg because of it and he throws off your sense of scale.
We expect young greg to be smaller, the gems are smaller than him, hence we think the gems are smaller. But that's just not how it is.

in both of these cases she comes up to pearl's shoulders.
and since pearl is sort of consistently the same size as greg, greg's shoulders as well.
Pearl seems to always come about right under Garnet's nose.
And Rose is XBOXHUEG.
Steven is a tad shorter than Amythest, and about on par with Ruby and Sapphire, about half a Greg.
The only reason it seems so odd with them pre-greg is because they're dressed for what their actual mental age equivalents are at the time, and possibly, though I'm not 100% sure on this, I think their proportions have changed to match. Not objective size, but size ratio of body features.
Combined with size comparison, it makes Rose look like a (9ft) 40-something mother, Garnet her 19 year old daughter, Pearl her nubile, early teen daughter (with an Oedipal complex of sorts), and Amythest her youngest at probably 6ish.
And as such, Greg, despite being 22, looks maybe 14 beside them, because he's the same size as Pearl, and has younger facial features than we're used to seeing on him. Which makes his crush on Rose seem almost quaint, like she's kindheartedly humoring him, if with some slight /ss/ undertones, and their interactions really sort of convey such until their attempted fusion and realtalk session, where she first starts taking his romantic intentions seriously.
