The hole will never close at any safe piercing gauge, so keep that in mind, it can be an upside or a downside. Got my PA 3 years ago after about a year of careful thought. Yes, it hurts like the king of all motherfuckers when it's pierced. Had sex for the first time about 3 weeks later. Not very long to be honest. Three weeks or so, gauging my PA was beyond easy.

I started out with two frenum barbells at 12ga., did it myself with a 10ga needle.
Several months later I put a PA in between the two with the same 10ga needle, but it was a bit too deep to ever find jewelry that would comfortably fit, so for the longest time, I didn't wear it. I don't recommend this configuration because while it looks and feels great, the pa will connect internally with the frenum above it, allowing you to pee and cum from 4 holes at once. eventually, I found large enough barbells and rings with the right sized curve to fit comfortably, and eventually worked my way up to a 2ga barbell on the PA, and 4ga. Frenums.
The larger gagues are much more comfortable to wear with much reduced pinching, and the added effect adds a nice swinging effect I previously was too high and tight to experience.
I've since then lost the frenum closest to the head from migration, but the pa and frenum are holding strong.
The PA at 2 ga. Is stretching the hole on its own though. I can't feel it stretching, but can definitely tell I can probably fit a 0ga. Ring in there now. I can leave it out for a month and it'll go back to a 4 or 6 ga size..

The 4ga. Barbells I got from painfulpleasures.com, and weren't too expensive. The 2ga. Curved barbell was $120+ though custom made and took a coupe of weeks to come in. I forget what length it is, but it's a bit longer than usual due to how deep my pa is placed.
Pic related-- last pic of mine before the frenum closest to the head migrated out. The segmented ring was just a bit too big diameter for the gaging up settled in, and more proper jewelry took too long to arrive.
It healed well though, the hole where the pa connected to the frenum internally is the new PA hole, it's exactly on center and just a little higher up. The old hole is still there, but goes largely in noticed unless messing with it directly.

Yeah, it's a shame it didn't last. By the time I figured out it was migrating, it was too late to try and save it as the distance in between holes was too narrow and off center to look good, so I just let it go all the way. Had the ring been just slightly smaller diameter or I used proper barbells instead of the ring, I think it would have worked out with no issue at all.
It made sex real interesting too, since the ring and large end ball of the PA covered up most of the best sensitive area of my penis, I gained some staying power, and when it took all the metal out, it was like having a brand new penis. Still is, which is why I keep the piercings, to change out and keep things from getting "old".
Here's a pic of what it looks like now; you can't really tell at all now that I even had a frenum there at all.
