DB is the largest German bank and one of the largest banks in the world. They got this big by investing in less than stellar places that other US/UK banks wouldn't touch. They are now saddled with bad loans and it is basically Europe's version of Lehman brothers, except instead of mortgages it is bad government debt (PIIGS). Germany through the EU has been gaving the central bank suppress interest rates so the governments can't default, but the problem is still there (zero growth, can't devalue currency).
Likely what will happen is the German government (and likely all EU member states) will bail out the bank. Whether or not it causes another Brexit remains to be seen. Germany certainly can bail it out if it needs to.

The German govt. has for years fought attempts at capital injection into struggling financial institutions, bailouts. In the case of the Irish, the Greeks, the Spaniards, and now the Italians.
