Las Vegas is run by businesses that the mafia used to run. Every ghetto now has a hundred payday loan shops running what essentially loan sharking. Even entire sports were created from organized crime. NASCAR is basically an outgrowth of bootlegging during the prohibition when hillbillies would drive cars faster than law enforcement was capable of reaching.
The mafia was made up of many people who weren't the scary, brass balls cold people Hollywood makes them out to be. They were made up of many attorneys, bankers etc who would wet themselves if they actually had to participate in violence but laundered the money, kept the cops off, etc. I would say only a very small fraction of the mafia were the enforcer types who would cause the trouble and take the heat, and most of those were just thugs hired and not actually "mafioso". Also a lot of the italian mobs were also made up of half Jews and many associates were fully Jewish as well.

In Italy they are an absolute cancer, even today. They're diminished, but they're still around and they provide absolutely nothing of worth. I used to be the "IT Guy" and business manager for a group of organized criminals that brought me into the fold after I did a spate of hacking attacks against an online service.
I'll debunk some of the myths:
1. They aren't as organized as people think they are. There like governments, everyone thinks they are capable of pulling anything off, in reality, there's alot of complications and problems.
2. Difference between petty thieves and organized criminals is the concept of "Loyalty". No matter what stupid shit you do, everyone has each other's backs.
3. Operating legitimate businesses is always a better option than breaking the law. Legitimate businesses can be scaled out hugely if done correctly. Illegal activity if its scaled out too big, brings too much unwanted attention.
4. The whole "In it for life" thing is a lie. They'd never pickup recruits operating like that.
5. Hollywood tries to romanticize the whole thing, likes it some sort of family. There is a "Family" kind of connection, I called them my second family on many occasions, but in reality its just a group of businessmen who don't mind breaking rules to get results, who look out for each other.
6. Despite the amount of times I asked, we never had a "Everyone wear suits" policy. The boss said "That was the 80's, just wear normal clothes". Sucks I wanted us suited up all the time.
7. The boss was openly gay to me and a few others in the group. You'd be suprised how many criminal sociopaths are into guys.
