what made Chernobyl so wacky is that they used graphite as the catalyst
so losing cooling water caused the reaction to compound on itself exponentially
basically if you had a group of scientists sit around and brainstorm on the shittiest design possible you'd come up with what they used over there

The primary coolant system has had a steady increase of radioisotope heavy minerals leaches from the walls of the reactor vessel as well as the control and fuel insert rods. There is no way around it with a system like this, and is the basis for a reactor's 'shelf life' because there's really no way to prevent this build-up in key pump control valves and subsystems. This leaching also eventually leads to compromise to the structural integrity of the vessel itself.
There is a steady increase of incidents because no more NPPs are allowed to be built in western nations. And with the crackdown on natural gas and fossil fuel plants, these decrepit reacors are being used at full capacity long after they should. Some are over half a century old now.

The RBMK reactor design had more than the use of graphite as the moderator going for it that caused it to fuck up. It was unstable at low power settings, had a positive void coefficient (the more "open space" aka steam bubbles in the core the more power it released, which can and did result in an exponential positive feedback loop), and the control rods were tipped with graphite instead of being a boron material the entire way through. Throw in some Xenon poisoning and dumbass operators for flavor and you get a 30GW power spike that blows the reactor apart.
RBMK is basically a really really shitty reactor design.
