Currently doing my masters dissertation on this so I might chime in.
Pakistan is increasingly shifting focus towards battlefield tactical nukes, used on front edge of battle only, as oppose to the traditional strategic nuke. This is far harder to retaliate against with MAD, and Pakistan will use this as cover to launch insurgency warfare in the event of a conflict.

No it is not. What that means is that they fire a mini baby nuke at an advancing army battalion and then India responds by glassing all their cities, while Paki nukes can't get past the S-300, S-400, and Israeli-Co-developed Air defense systems.
Any nuke of any size at any target will be met with disproportionate mass retaliation that will effectively cost a minimum of +100 Million Pakis upwards to 180 Million. After that point India will probably annex the baron nuclear wasteland while the survivors slowly die off of radiation poisoning and mass infertility.

What you are saying is correct on paper. India formally abides by No First Use, meaning that theoretically its only response to a tact nuke strike would be all out retaliation.
In reality however, if this was the case India would not have developed their own tact nukes.
Also, air defences are not foolproof. Pakistan has other delivery methods besides ICBM. For example shoot and scoots, or suitcase nukes given to insurgency units. India could not be certain that it would escape unscathed, and so it would illogical of them to purse their Massive Retaliation doctrine.

Not really. If Pakistan responded with Nukes they would at least glass the capital of Islamabad, and then probably the only Pakistani port city of Karachi.
In every logical scenario escalation continues until Pakistan gets destroyed and India maybe takes a few hits from baby nukes on their advancing military columns.
Pakistan has no way of winning, just sacrificing millions of their own citizens to inflict some minor damage to India.
