There are probably ways to do it in Photoshop but the classic way is to use a zoom lens and zoom in on the subject during a long exposure.

It's some tacky lens called "Lensbaby the Composer". Shit is wack. Some knock-off tilt shift that's probably more of a selective focus lens.
Could just feather a circular selection around whatever you want to be in focus, then select the inverse and apply a radial blur in Photoshop.
