The artist gave it to the band as a mock-up and asked for feedback, the band liked it so much they insisted on using it as-is. The artist really didn't want them to but couldn't convince them, so no other version was made and the artist chose to be uncredited to save the embarrassment.

Reality is pretty good, a solid record, nothing amazing although New Killer Star is one of my favourite Bowie tracks.

That's part of the AESTHETIC nerd.
Some background: this album was released by Geoff Tate's band calling itself "Queensryche" during the time window after the band proper kicked him for being a violent, alcoholic asshole and before they got a court order ordering him to stop using the name Queensryche because he didn't own it anymore. The "FU" is in reference to this whole debacle, making this album cover not only terrible, but also a tangible product of a bald grown man in eyeliner throwing a hissy fit because his friends wouldn't put up with his bullshit anymore.
