Anon was hit by a car and was left in a coma for two days, but in his mind he was teleported to Equestria for six months where he lived his life as he perceived it. Then, after leaving the portal and awakening, he is informed that he simply made up his adventures and the time dilation in his head. So in actually, nothing happened but he thought it did and he made a connection with fictional characters he made in his mind.
As an extra note, the author mentioned the doctor's distress with "As the doctor reaches the door, he slowly closes it, looking at you with eyes filled with sadness" indicating that the doctor understands his pain in some degree. “I’m sorry, but they’re not real...The faster you can accept that the fast you can get better.” My take is that the doctor hates seeing another mental patient being destroyed by emotional distress (and the protagonist is hinted as being mentally unstable with the straight jacket he is wearing), or the doctor also has a similar emotion for the character the protagonist "imagined" which would indicate that they might be real in some sense, and the doctor perceived them, but received backlash when opening up about them.
I like to think that when Anon dies he will return to Equestria and race Rainbow Dash again.
