Request it from your primary doctor or find another clinic that does it.
If you get blood work, you might need to specifically ask for them to check your hormone levels. That's how it works for me but I don't know if that's the case with everyone.

Then you're getting the right stuff. You'll likely need to move up to 4 mg, or even 6 eventually, but starting low and building to the correct dose is probably best practice.
