Theyre less effeminate as children and teenagers than the tru trans, so they end up fooling themselves and others that they are cishet, and never get to confront their inner femaleness early on. The "true transexuals" thing is just a meme mostly local to here.
Even the people that came up with the hypothesis of a dichotomy along the line of orientation defined those who like men not as heterosexual but as homosexual transsexuals because they considered them a type of super-gay guy basically and they considered both groups valid in transitioning.
