The most that would happen to a 12 year old is being put on hormone blockers, that are temporary and reversible, to give the kid more time to see if it's a phase or not without permanent changes happening to their body (remember, even natural puberty is permanent). Actual cross-sex hormones wouldn't happen until they're 16 or 17 with parental permission and surgery wouldn't happen until 18.

I've (sadly) wasted a lot of time reading blogs of "detransitioners," and most of them just took T for a while. Some got top surgery and/or a hysterectomy.
