You'll talk about why, price, and he'll tell you that you need a letter of recommendation from a therapist to make sure you are doing okay mentally and getting the surgery for decent reasons. They won't decline you if you aren't trans. They just need to know this isn't a snap decision made last night, not insane, and that you have a decent reason for wanting it.
After you had the consultation, then got the letter, call back up your surgeon for another consultation visit and you'll work out an appointment date together.

Really, anon's suggestion is the best way to go about it, the problem is more a matter of where you live than anything else. Some doctors will be like "yeah no sweat, it's your body, no big deal." Others will say "no way that's not necessary and hence unethical". There was a female-to-androgynous transexual from Cambridge (or somewhere near by) that was able to convince a doctor to perform a mastectomy and hysterectomy.

Of course, it's still an unusual request. The medical industry (in the western world) has a tenuous relationship with elective surgery, they kind of embrace it but only in the really common cases so that they don't have to think too hard about the ethics.
As more cases emerge though, this will change. If things like this are really important to the person in question, they should push as hard as they can for it. It will set a precedent and make it easier for others int he future to get access.
