They do it for some very tall women, but it's not common. For perspective, the women in question are like 6'4"+ - all these MtF's in this thread around 5'11" - 6' would be turned away like any other woman of the same height.
Bone printers do exist however, and this would allow you to replace or reshape any bone in the body. The result would have better proportions than simply cutting down long bones, and could be completely customized. Every bone in the body could be reworked, and made to appear female.
Also, it's not completely cosmetic - many women have trouble with narrow pelvises. MtF's will probably benefit from modifications to the hips that will innately widen the area, and if you're doing that you might as well modify the pelvis to have a female shape and tilt - and this could be chosen by the patient. Anyone could walk out with a wide, long pelvis that forces the spine into a very exaggerated state of lordosis.
The change in the bend of the spine is going to remove several inches when standing. Chopping down vertebrae or completely replacing the spine could remove even more - and this would solve the aesthetic problem of MtF's having long, thick necks. You might as well shrink the ribcage while you're at it, at which point the shoulders shorten as well.
Many people complain about their big heads, but really they have big faces - a small face on a huge skull and tiny, short neck looks perfectly feminine. You'll probably want to taper the limbs a bit and proportion them properly, but most MtF's will benefit from their long, manly legs. The hands and feet will probably be the most difficult part.
It seems like a synchronicity to me that MtF's are the best candidates for anime-proportioned bodies, and they all want to be kawaii shoujo.
