Al spitzer bullets tend to keyhole hitting soft media- the wight of the bullet is largely to the rear. When the bullet turns sideways it can tend to break at the cannelure- so now you've got two wound tracks.
My understanding is some of the ammo produced by the South Africans in the 70's had a similar issue due to a steel penetrator in front of a lead core.
