No one ever lies at MEP med exams. Also, people drown and are brought back in combat diver and BUD/s all the time. For guys not in the Navy, dive school is one of the most hated, tough and dangerous schools they can attend.
Every CCT and every Soldier Ive ever known who went through it says combat diver was the least fun theyve ever had.

The instructor is accused of splashing water and dunking him twice, during a full load tread test, so the kid has asthma and an enlarged heart and died. The instructor didn't hold him down until he stopped moving. He dunked him and splashed him.

Certainly instructors are to blame, at the very least they were negligent to recognize the trainee was in mortal danger.
However the sailor most likely was not willing to quit, and bit off more than he could chew. Still, the training cadre should have disqualified or recycled him if he wasn't able to keep up.
