it's hard to fuck up a walther PP clone and the makarov is a superb PP clone. generally, gremlins are easy to sort out by virtue of it's simplicity.
that said, it's both surplus and slavshit so there's always the occasional lemon or case of outstanding abuse. from a design standpoint point though, it's probably the finest blowback service pistol out there.

I'd say an HK USP, glock 19, FNS-9 (these are relatively new but no bad stories as of yet, and FN makes a best barrels), cz75, browning hi power, ruger p89 (supposedly had exceptional reliability, and were very popular in the 90's)
You also want to look at commonality when selecting an SHTF gun. Caliber-wise, don't be an autist that thinks you're going to scavenge parts and mags. If you really think that's going to be an issue buy spare parts and mags.
