I always imagine them creating a black hole at CERN. They figure it's too small and too short-lived to be of consequence, so they create one. Instead of being small though, it's just above an unknown threshold and it sticks just long enough to drag a few atoms in. They see it's not going anywhere, so they try to contain it, but it just keeps consuming subatomic particles.
After a few days, they start to sweat. They can't see it, but nearby technicians start to feel a little wonky when they're standing by it. Just a slight sense of vertigo. Nausea makes it difficult to stand by the isolated anomaly.
They can't give up, but it's growing at an exponential rate. The vacuum chamber it's stored in starts to collapse, and they all decide it's time to admit failure. At a wopping 10lb of mass, it's not a threat yet, but it won't stop feeding.
That or they accidentally create a self-replicating grey goo that utilizes nearby molecules for it's propagation.
I dunno, both would be cool.
Personally mind-shattering earthquake followed by a badass poison-spewing volcanic eruption would be cool. Could you imagine what would happen if "the year without a summer" happened today? Economy gets raped. If the eruption were worse, then the damage would be multiplied. I don't need a Krakatoa (but planes getting knocked out of the sky by a shockwave would be crazy) but a global cooling might do serious damage. Not world-ending though.

Ebola or a type of modern black plague is what everyone always suspects but I'd argue something like a airborne or insect carried(think Malaria) form of super AIDs would be much more lethal.
