I'm watching MASH and one common event in a number of episodes is that they are in the OR for 20+ hours operating on wounded folks.

I thought the purpose of a MASH unit was to stabilize the patients and then ship them to a specialty hospital for further care. Does the show depict MASH units from the Korean War truthfully to some extent, at least in how they operate?

Which is exactly what they do. Do you think there's only 10 casualties for a battle? They just don't always show the hundreds of casualties they get over the course of a 5-6 hour offensive. There's also cases where they might have to spend a lot of time on a single patient to get him stable.

How do they offset fatigue exactly after 20+ hours? From the shows standpoint they have 4-5 surgeons. Just breaks every few hours or so for 20 minute times?

Coffee. Lots and lots of fucking coffee. When there's a few days between such exertion, you can deal with it without too much issue. Surgeons today do it a lot as well, look at the heart transplants and shit that take the better part of a day.

I actually think it gets better once Winchester gets there. It gets a bit darker but they still have enough humor to make it tolerable. My parents actually thought Scrubs reminded them of it, with the mix of humor and serious topics.
