I've heard scattered talk that there is a truth about nuclear weapons known by the people who design them that they refuse to publish because they don't want the people who control them to know. The fact of the matter is that war has changed. 1984 was extremely prophetic in some regards. The T-72 is currently being phased out. It is being replaced by Leopard 2s and PT-91s. I guess you could call a PT-91 a T-72 if you wanted to, but it's only slightly worse than the T-90.
The Polish military isn't a pushover. The amount of well trained tank crews ensure that Russia cannot blitz its way through Poland yet again.

Assuming that side B can determine early stage missile launches, and accurately determine the launch origins then yes; however, in the case of Russia they currently do not have a functioning early launch detection system as their new system is continually delayed and their old system pretty much doesn't function at all. This means if the US strikes first, Russia will have no idea which missiles have been fired, and consequently will be unable to refine their aim based upon which locations have fired. In the inverse case, Russia's assets are more mobile and thus we really can never know all or even most of their locations.
