nipples tend to look better when you start with the darker tone of the nipple blocked in way over the size of the areola an then go about painting the regular skin tone back over it till you get the size/shape you want
Essentially this one or something close to it.
The square setting is one of my own design, however. Which is why the quality is at 4, it's also a good idea to use stablizers with any boxish tip to create a smooth path rather than a choppy one at ever turn.

It's just rings of slightly different shades blended and some hard edge speckles. You can do that with any brush.
