The anime is garbage, but the VN is great. The sex scenes are hit or miss depending on if you're into that scene's fetish or not. Only a few of them are really disgusting and overall it's a lot tamer than the hype would have you believe. None of the gore involves sex and in fact all of it is contained in very brief shots from bad ends.
It's really worth playing for the storyline. It's far from perfect, but there's one character and one plot twist that makes the whole thing more than worth it.
