It is sort of the same things, only a different reaction. They are all centered around being overwhelmed by pleasure. However, regular ahego is is loose, relaxed and generally meant to show how they can't have lost control of themselves. Rolled back eyes, hanging jaw, tongue stuck out, body just kinda flopping around, etc.
This is very tense comparatively. A clenched, jaw, wide eyes, focus and tension throughout, etc. This is trying to cope with such pleasure through normal trauma mechanisms of straining one's body to protect from shock, the same as what one would do to sudden sharp pain.
