Considering that twins are genetically the same, they'd possess both dominant and recessive genes as the other one. Those would pass down from both sides of the family.

As the DNA being recombined in both pregnancies (of the two couples) is virtually the same, then if both happen to get the dominant genes in most characteristics it isn't even that statistically unlikely that the resulting kids will inherit a very similar makeup without being siblings.
And as we know, siblings (even if not twins) can be born incredibly similar. It is not at all unreasonable that, under the premise the manga established, the result could be two uncannily similar girls.
They would never be the exact same (because even twins aren't exactly the same genetically, especially once adults) and the odds that they would recombine the same way in the different couples are low, but the odds that they would recombine similarly enough for the major features to be strikingly close is actually pretty high.
Certainly possible that you wouldn't suspect it's a twin (and certainly not suspect this rare scenario) but simply feel something is 'off' if you were meeting them separately one after the other (though you could definitely tell who's who when together).
The fact that they are drawn the exact same way though I simply attribute to simplicity for the artist's drawing, but the idea portrayed is simply that they incredibly alike.
