Good smoking fetish hentai is really hard to find. Unless I'm mistaken smoking (sexually anyway) is associated with BDSM in Japan. In anime it's usually used to show characters are older, more mature, jaded or crude/cruel. It's a culture thing.
As previously mentioned Shadman does some awesome smoking fetish western stuff, if you can abide his penchant for assholes and futa.
