I have and have always had this nipples even though I've always been, and still am, really skinny. Is it gyno or it's just the way my nipples are?

Well I can feel something right under my nipple but it's the same feeling as in many other parts of the body between the muscle and the skin, probably fat or something.
puffy nipples is not gyno
some people have a bit of fat behind their nipple which isn't mammary tissue ie: not gyno

Your best bet is to get bigger - when you are skinny mild gyno really stands out, if you put on some muscle, it won't be nearly as noticeble.
