I've been duck taping my squat plug lately. It's super uncomfortable, but it works. The only good alternate I've found is a super tight thong of some sort that has the line running through the crack, or briefs with really tight leg holes: though both those are uncomfortable in their own ways.

The only one of these that's correct is the hat thing. And it's not specific to the gym, you just don't wear hats indoors. It's common sense.
Running shoes - I get why you list this, it can make certain lifts (e.g. Deadlift) less efficient but there's really no "rule" about this. It doesn't hurt you if I'm wearing the wrong shoes.
