The trenbolone enlarged his heart, and clenbuterol is known for causing heart valve defects. Combining the two is a stupid idea because one makes the heart more muscular, and the other makes the heart work harder, resulting in even more musculature of the left ventricle. Dont even get me started on the DNP. He was a nice guy for sure, apparently very humble, and he was pretty intelligent. But he had insecurities that drove him to be too ambitious with how hard he pushed his body. So yeah, it was tragic, and probably caused by him being teased for being really skinny in his teen years. The first thing that comes to mind regarding sudden death of a fit young person not otherwise using drugs is hypertrophic cardiomyopathy. Unfortunately, 'preexisting heart condition' is far too vague to really narrow things down here. Bicuspid aortic valve leading to aortic stenosis is another good possibility that the AFP article brings up, but all doctors know that strenuous exercise is totally contraindicated in aortic stenosis and he would've gotten an aortic valve replacement.
Drug abuse would be the first thing on my differential if I was seeing this as a case. We deal with numbers in medicine, and prevalence of drug use/abuse is going to be orders of magnitude higher than the prevalence of SCD risk factors, even in an athletic population.
