My BP is 155/88

How do I lower without meds?
>>
>>35638353
DASH diet
https://en.wikipedia.org/wiki/DASH_diet#Diet

Basically high potassium, fruits and vegetables, low fat or nonfat dairy, beans, and nuts. Poultry

How old are you? Has it always been this high?

I fell off my bike a week ago and couldn't move my shoulder. Now I can, but a lateral raise with my shoulder blade tucked in still hurts.

Go light for now. When you get an injury like this, range of motion comes back first, then pain goes away. It still needs to heal fully.
>>
>>35638362
28. Not sure. Its been up since around 20
>>
>>35638423
Diet and exercise can lower you down to normal range. If this is not working, meds are not a bad option. There are a lot out there with minimal side effects. You really don't want high BP for too long.

How do you feel about hospital power structures? Are you willing to get consult from your colleagues? Are your nurses willing to correct you/ask questions or do they just do whatever you tell them regardless of personal opinions?

Recently read an excerpt from Dr. Hilfiker's book "Healing the Wounds" for Medical Ethics and I was pretty surprised at how guarded and egotistical a lot of doctors can be. Not saying you are, but I'm just wondering what your experience is.

Rheumatoid or osteo?
Acetaminophen is good if you need relief, Its much better tolerated and less toxic.

The key thing is DO NOT rest the joints when they hurt. Exercise and movement has been seen to increase cartage growth in joints,

>How do you feel about hospital power structures?

They are great. We are taught that patients get the best care from large health care homes/systems.

> Are you willing to get consult from your colleagues?
There is no way I can keep up with current research/protocols on every specialty. I need to consult all the time. Mostly just to cover my ass.
>Are your nurses willing to correct you/ask questions or do they just do whatever you tell them regardless of personal opinions?
I like to invite all staff to practice at the top of there license. I will have a couple nurses that I trust go ahead and treat according to protocol before I even put orders in. They can be great, but you have to be able to trust them. They are very good to me in return.
>>
>>35638686
Not for osteo arthritis. In osteoarthritis, the pain is more due to mechanical wear and tear in the joints. While it is true that asprin or other drugs may give slightly higher pain relief, acetaminophen should be the first thing you reach for because of its lower toxicity and less side effects.

Their is no good treatment for back pain unfortunately. Rest does not show to help. Exercise and stretches do not seem to do much either. I usually just have to tell patients to go about their day as usual and take an NSAID as needed. It should go away within 2-3 weeks if it is musculoskeletal related. If it doesn't, you need an xray, then MRI

Coconut oil has one of the highest saturated fat contents of any oils. Even more than butter. Saturated fat is proven to raise LDL (bad) cholesterol and lower HDL (good) cholesterol.

HOWEVER

This has not yet been seen in people eating the saturated fats in coconut oil. So we honestly do not know how good or bad it is at this point. We do know that virgin olive oil and canola oil will raise HDL and lower LDL. So I would stick with those and avoid coconut oil for now.

Avoiding all poisons but it really hurts every time I eat. Should I eat less or something? Already on pretty small meals. 70 grams of fish makes my side hurt :/
Any pain killers that I can take safely?

I dislocated my shoulder doing a muscle-up about 3 months ago, got an ultrasound the day after, and was diagnosed with a irritated bursa
After treating with anti-inflammatories for a little while I started doing gentle rotator cuff rehabilitation exercises, and the pain is completely gone from my day to day life. Every time I try to deadlift though, it feels like it's gunna fall out of joint again, and hurts like it did in the first week.
I'm able to OHP 70kg again, down from 80, but doing that weight irritates the shoulders afterwards. Basically I'm stuck doing legs and OHP, and I'm scared of doing bench, because that was the exact angle that my shoulder dislocated on, and it feels 'loose'.

Wat do? I'm honestly considering roids for the duration of my rehabilitation, because I feel like I'm just gunna keep losing strength.

I've just started running and have just done the first week of C25K and about half way through my second and third runs my right foot got tingly/numb. It felt like it was most of my foot except for the top part. When I finished the third run I could feel a slight tingling sensation in my left foot as well.

I tried loosening my laces after it happened the first time but it didn't help. Should I just go to a doctor or try replacing my shoes? Is there anything else I can do to prevent this?

Also should I be running if I'm 89kg at 5"8- 5"9? I don't want to unnecessarily damage my joints.

Its gotten much better since then, by my lower back still hurts sometimes. what do you think would cause this? I think the injury came from a twisting motion.
also, I started going to the gym and every part of my body hurts like hell. How long of a break should I take between gym days.

If food is causing "liver" pain, it might be due to gallstones.

Your gall bladder is very close to your liver, and releases bile to help digest fats. With gallstones, some small stones form in the gallbladder and get stuck in the bile duct, causing pain whenever your gall bladder wants to release bile.

Cut fats entirely out for a meal or two. Then have a fat full meal. Then report back.

Hello doc

I have continuous pain in an area similar to pic. It usually hurts like a bitch during squats and deadlifts. And it also hurts overall all the time. This has been for around 2 months and it keeps getting worse. What do?

Any weird urine? Does it hurt when you pee?

Lay off the squats and deadlifts for 2 weeks and see if it gets any better. If it does, you are squatting wrong. If it does not, go in to see the doctor.

Doc I get pain in my wrists right after re-racking the weight during heavy bench.

135lbs is OK. 185lbs is not OK.

I have the bar firmly in the low part of my hand, no wrist bend whatsoever. Wrist is in alignment straight down the arm I've been watching my form every time.

It's like the pressure keeps hitting the wrist joints and it only hurts 3-4 seconds after relieving pressure. I've never had this happen before it just started 1-2 wks ago.

Any advice?
>>
>>35640719

Thanks doc.
>>
>>35640748
Sorry no idea on this one. Joint and musculoskeletal pain is difficult to pinpoint a cause. Could be a minor tendon strain from a week ago that you do not notice until under load.
>>
>>35640814
Should I stop heavy bench for a couple weeks to see if it improves?

Similarly if I keep benching will it just get worse & worse till I end up in physical therapy?
>>
I eat 600g of tuna a week

is that bad? I'm 19, 150lbs.
>>
>>35640831
Is it getting worse or better currently? Do you have full motion? Any swelling?

If it is a strain, rest will help it heal faster. It may heal with heavy benching as well but it may slow down the process.

This is all if it is a strain. (probably)
>>
>>35640894
No swelling but it does hurt periodically throughout the day - specifically after a bench day.

I benched again today and I'd say it's worse than last week. So I'll give it a couple weeks off and hopefully things improve.

Much appreciated
>>
>>35641098
Yeah if it is getting worse in intensity or frequency then definitely hold off on the benching. Ice might help

Here's your second opinion:

>155/88
That's way over pre-hypertension. Secondary hypertension reasons (kidney disease, pheochromocytoma, etc.) should be eliminated. If it's essential hypertension, your GP should prescribe you ACE inhibitors. If you don't take care, you may die of heart failure at a relatively early age.
>>
>>35640192
>>35640238
Actually, it depends on the angle of scoliosis and one's age. Primary surgery indication comes with larger degrees. Lesser scoliosis can get better in young age with swimming, pull-ups, pilates, etc.
>>

My resting heart rate is high, as in usually over 100bpm. My blood pressure is fine, though.

There are these tiny red spots on my balls, about 4-6 of them, they are tiny lumps. Sometimes they get really itchy and sometimes I feel nothing. I'm afraid they are the same thing as the fungal infections that I got on my ankles and hands. Should I get my balls checked? Unlike my legs and hands, the lumps on my balls didn't get worse and start leaking liquid. However the lumps on my balls itch the same way but sometimes the itch goes away for a long period and sometimes it returns. I think it could be related to sleep deprivation
>>
>>35641779
ECG may show hypertrophic heart. If your father has hypertension, so you have genetic predisposition. < 140/90 blood pressure is considered prehypertensive stage, 140 - 160 is stage I hypertension. ~160 when people gets two kinds of anti-hypertensive drugs. Go to an internal physician, tell him your symptoms. He would check you kidneys. No smoking, no excess alcohol, regulate salt intake, exercise, don't gain fat are your best options.
>>
>>35641893
What? How are you saying that bp 135/55 = hypertrophic heart?

Should I do some kind of course, keep doing as I'm doing or is there something else you recommend?
>>
>>35641818
That doesn't seem to be physiological tachycardia. Beware, you may die of tachyarrythmia. Go to a cardiologist; echocardiography should be useful. Genetic predisposition plays a role, your ancestry may be sensitive to cathecolamines, etc.
>>
>>35641818

Try stretching moves to relax your tight muscles, you can find lots of videos of that. If you accidentally hurt yourself in excercise, go to a physiotherapist. It may be about a muscle tear or your vertebrae, etc.

Ey Doc, here's an off topic question.
I shook an old guys hand after assisting him at work and shortly thereafter I noticed a small amount of blood on my hand. I assume he did something and hadn't noticed yet, and I couldn't find any cuts to show it was my own. I believe the biggest risks are HIV / Hep B, got my shot maybe 20 years ago for Hep B, and I have no idea of this mans medical condition.
I know you can go to the ER and get meds for HIV, but it being a small amount of blood and on chapped skin at worst, is it realistically worth going through that month long treatment?
I noticed it within a minute of it happening likely, and immediately went and scrubbed the crap out of my hand with hot water and soap.
Should I even worry?

Also, I believe it was 155/88 in his first post. When he said "135/45-55" I thought he was referring to his systolic pressure. Diastrolic pressure from 45 to 88 is likely pathological.

No, that was a different guy. I think I have left ventricular hypertrophy. My diastolic pressure is low beyond the charts (highest it's ever been was 65), but have systolic hypertension, apparently. Sometimes it's 120, other times it's 140-145. I have mentioned it occasionally, but docs don't seem to care, they assume it's a clinical artefact.
