That seems to be an unfortunate side effect of genres intersecting. If it were a proper, well-funded institution, it wouldn't have that dark gothic charm, and half the time Batman is the one taking a criminal there which makes it seem like he trusts the place when he clearly shouldn't.
If the place worked, it wouldn't fit the show, and if it were properly excused as part of Gotham's mixed criminal justice infrastructure, then Batman would have less business to do there, both of which would probably hurt the show more than just accepting the "revolving door" trope.
It's not so much that the people are bad at the job, it's that the job itself is impossible.

Security? They try at least but these guys need to be in something besides cells that just nullify some of their powers and more about ensuring that person who is usually smart/clever as hell doesnt get too much into a rhythm because the criminals are just waiting for a chance when you lower your guard
The people who release them and call them sane? I think most of the time there is some corrupted official behind their release or someone with connections
Rehabilitation? Yes, sometimes you forget it is a nuthouse and theyre all treated like sane, albeit very dangerous and sans, criminals
Outside of someone say Zsaas level no one tries to actually cure people not even the extremist types like ivy, two face, or riddler who at the end of the day just need a nice prescribed dose of "calm the fuck down" that they should take after every meal
