I don't expect Lance to change positions soon. He's a little hotblooded-ish early but so far the show seems to be moving him away from the direction of the Red Lion, not towards. And we don't know the Blue Lion's special personality trait, which is probably more than just welcoming new pilots. So it's either Keith graduates to the Black Lion and Allura takes over as Red which is extremely unlikely, or Keith stays but they find a way to make Red becomes the head like Force, or Allura takes over for Black which is also not unlikely given interviews suggesting that leadership is a skill she'll develop over the series.

He's poisoned by Haggar's attack and it makes him weak when he exerts himself too hard in battle. He's not in any immediate threat of death but it's a Sword of Damocles hanging over him.
Rather than stay back and rest, he keeps on fighting for the sake of his team.
Eventually one day he knowingly pushes himself too hard, because if he didn't Pidge would have died or something.
He comes back from the battle, severely injured but puts on a brave face so his friends stop worrying, and then BAM-pineapple salad.

My bet would be that he was a Galra experiment to create the ideal Paladin to serve under Zarkon, essentially a designer baby with all the best bits of DNA for the job. This would include Altean DNA because the Alteans seem to be closely tied to Voltron. It's their castle/flagship that was specially designed to serve as a base for the lions after all. Something something lab explosions, Keith is ejected and lands on earth. Because of his Altean DNA he changed into a human without knowing it and was raised in foster care. His latent programming made him subconsciously desire to return to outer space and find his lion, hence why he joined the academy and was able to find the Blue Lion despite being meant for the Red one. There will be a "Luke I am your father" moment when we find out who the donor of his Galran DNA is. It's likely either Zarkon or the traitor who helped them escape by bringing down those shields.
