Honestly with a rogues gallery there's pretty much never a "greatest foe," because there's always a particular villain that can claim that title in a certain way while another does it in a different one. Like how by various metrics someone could claim Lex Luthor, Darkseid, Braniac, or somebody else I've missed is Superman's "greatest foe."
Among other things there's the tendency in a medium with this many writers for writers to have a pet villain that they build up while shitting on a different nemesis. The Joker's just unique in that people keep trying to top each other in how crazy and repellent they can make him, which is kind of annoying because they already sorta hit the space where the character works best with portrayals like BTAS, where the Joker is obviously crazy and difficult to predict because of his demented sense of showmanship, but is also still, y'know, a criminal rather than a natural disaster and has a life outside of Batman that means he really would just shoot Bats if the opportunity presented itself and be fine afterwards.
Admittedly I realize I'm probably biased towards that interpretation because it's the one I was first acquainted with, but I still feel like it's the one that works best. The character's still DANGEROUS because he's crazy but also fairly intelligent and very ruthless, but BTAS and the Justice League also remembered that Joker was a guy in a purple suit who was not going to do too well when a superhero got their hands on him and someone Batman knew well enough to anticipate and outwit.

If you want to TRULY blame a story for changing the Joker from The Clown Prince of Crime, into a psycho evil killer, go back and read The Joker's 5 Way Revenge from Batman 251 in 1973.
