In the Crossed books theres extremely rare cases where a person with super rare JEEEAAAAANS catches the virus, but doesnt go totally feral. They still go crazy, but retain basically all their memories and some of their personality. They just end up like a retarded physcho version of their former self.
They usually end up as leaders of colonies of other crossed people, since they are accepted into the group, but have enough intelligence left to run things.

Of course, given how quickly it spread, how potent it is, how specific the symptoms are, and how the infected people don't do anything natural and instead go out of their way to maim and torture people in the most cruel and downright ridiculous ways possible, the virus doesn't have much of a realistic scientific basis.
So, there's a subtle, underlying implication that the virus is some kind of demonic entity or supernatural curse, or simply a plague from an angry god. Because otherwise, it doesn't make any sense.

Expanding on the "angry god" theory, it's a pretty safe bet that Ennis set the whole thing up as yet another thinly veiled slam at Abraham religion. But of course, thinking logically, an Abrahamic deity would detest inequity, so why bother making a curse that causes more of it? Why not just blow the whole world up and start again? Certainly you can't exactly predict or judge the motivations of an all-knowing god, but even within the context of the old testament, it doesn't match up with anything God would do.
The more logical (and IMO far more interesting) conclusion is that the virus was created by some kind of Lovecraftian horror, like an Elder God or some kind of horrible sentient fungus at the earth's core. That would be a good justification for the fact that the Crossed seems to have both a biological/body horror aspect and a supernatural one.
