It's a send up of porn. Most porn treats women very poorly, but people don't care because they see women as objects. What MacFarlane has brilliantly done is put into the role of pornstar a straight white man, someone the audience will care about. That way when they see him degraded they feel empathy towards him and realise just how wrong the entire situation is. This is turn leads them to having empathy towards the women who are put into such situations, and they start seeing women as people instead of just objects.
It's very cleaver satire designed to both amuse the audience and send a social message highlighting the problems of our world, and further proves what a true artist Seth MacFarlane is.

I know /co/ will tell me Family guy was always shit, but the season I REALLY liked was the third season. They got rid of the cheesy "sentimental" moments, the writers were starting to rely on cutaways less and build more coherent stories, and the dialogue was getting more witty and rapid fire. It seemed to be a perfect balance of what they're doing now and what they were doing before the first cancellation.
Unfortunately season 4 kinda broke the momentum, and now the show is just dull. I mean, Family Guy still gives me the occasional chuckle, but there are no episodes that really stick out for me anymore. It's all a black hole of mediocrity.
