Why are Aunt May and her husband ancient when they're the sister in law and brother of someone whose child is a high school student?
Significantly older siblings (say, Ben's the oldest of 7, and Peter Sr. is the youngest, or an equivalent age gap for some reason), and/or Peter's parents waited until they'd finished their lengthy education careers to have a child.
Could also be that she's a great aunt (grandparent's sibling) and it's just too much of a mouthful to say all the time.

I don't have any nephews. My sisters only just started talking about having kids.
My Grandfather is...77 this year I think. His oldest sister would be 103 this year if she hadn't have died about 5 or so years ago. He was the youngest of 4 I think, maybe more. He has multiple nieces and nephews who are older than him. My mum never really had proper cousins on that side of the family, they were basically her aunts and uncles.
If Trouble were actually canon, then May and Ben would be candidates for worst genetics, aging like milk in just fifteen years.

Large age gaps between siblings used to be much more common than they are today. Having a decrepit, widowed aunt wouldn't have seemed unusual in 1962 when Spider-Man debuted, when families of ten or more children were common and the eldest might be twenty years older than the youngest (and then factor in the possibility of that youngest having a late child...).
Also, people used to just plain look older at younger ages. They didn't take care of themselves, smoked a lot, drank a lot, had debilitating childhood illnesses that aged them, ate less nutritious foods. If I were to compare a picture of my grandmother to one of my mother at any given age after about 30 (and my mom turns 65 next month), I'd take my grandmother for being considerably older.
