The Joker once went into Carmine Falcone's house, killed his bodyguards and threated the Roman himself and then just walked away. Honestly, it's useless to ask questions like this. The comic book status quo pretty much allows the Joker to be essentially immortal no matter what. This is a list of potential scenarios to eventually happen to the Joker if DC were more realistic about it all of the enemies he's made:

Isn't this kind like asking why any group of bad guys hasn't killed the Batman? I mean we have to take into account that the Joker is pretty much batman's opposite but equal. He's obsessed with justice being done the "right" way. He's not supposed to kill the bad guys, but just see to it that they're reasonably stopped and are handled by the justice system as fairly as it could be.
This is also unrelated, but this is why the Punisher is the Anti-Batman in almost every way. Despite wanting to stop heinous criminals as well, he sees the justice system as bullshit and a failure, and thinks that his justice is just and the only way to get rid of people as bad as the Joker. However, Batman knows how corrupt the system can be, but even he will still continue to put his faith in it, hoping that it can do what it was intended to do.
