It was the Iron Man rip-off guy and the actress she took hostage. I think she developed Stockholm syndrome and broke him out of jail, and they were both on the run from the law. Ben and the gang were chasing after them, and found out about the actress coming from a broken home. The villain even gets plastic surgery and outright kills the surgeon after that.
There was little to no alien stuff, technology, magic, etc. It seemed more like a cop drama episode than Ben 10. It was weird and dark, but also dumb.

they fixed a lot of plotholes in this made by unnecessary changes in UA, like it never made sense that Kevin was an alien or that his dad was Plumber, if any of those where true Max should had been able to recognize him in OV and the Omnitrix should had been able to analize Kevin's dna, the mutant origin always made more sense.
I really don't like how they regressed the show for OV, especially as the writers were layering some complex undercurrents into some earlier episodes.
Like how it was sorta implied that Charmcaster might've slept with Darkstar, particularly with the final battle starting in her bedroom and Gwen's reveal that she knew his real name. That she was on more intimate terms with Michael even after Charmcaster had given him everything. You really start to feel bad for Hope.

Even so, even relative simplifications and "dumbing down" of artstyles don't have to mean a worse show. Mystery Incorporated is very different from Be Cool Scooby Doo but it's still fun to watch.
