He's very sensitive about his appearance and abilities in Nu Canon. In Lords of the Sith he only trusted his personal guard and wouldn't reveal his powers to even Twi'lek villagers with barely any tech unless he was sure they'd all be murdered. He doesn't abide witnesses.

The Emperor didn't merely trust faking out the Bothans to lure in the entire Rebel Fleet to Endor he actually went undercover as Agent Pruneface to ensure the Alliance acted precisely as he wanted them to. It all makes sense now.

It's possible (just suggesting this, I realize it's fairly silly) the Emperor only uses his fake image in backwaters of the Outer Rim to project strength in areas the Empire hasn't fully consolidated it's power. In the Core worlds he may feel secure enough, and he certainly has enough Imperial presence, to appear as himself.
Frankly I don't know. It seemed weird in Rebels that he was using a disguise, but it's entirely possible that his speech announcing the Empire to the Senate was entirely off-camera so I guess only the Senators and their aides know about how deformed he is? Frankly it's one of those things I just shrug and roll with.
