Stokoe's got such an insane attention to detailand apparently he's hypercritical of his won workthat simply put his output is too slow for him to do regular work a lot of the time. Apparently he's thrown out nearly finished drawings because some small detail was off.
Also, on a side note, the guy is almost disturbingly talented. I mean shit, he can draw with either hand, and without using his fingers. There's pictures of him drawing using only the gab between his thumb and forefinger to guide the marker, it's crazy.

Did he do the coloring for that illustration too? I'm always disappointed when the first couple pages of a manga volume have some colored pages, but they're poorly and/or lazily colored. This I actually like.
Although I can't imagine he'd have time to work on a Western book with how demanding manga schedules are.

That's easy to answer. It's because Stokoe wants to just focus on what interests him rather than paychecks. He occasionally takes licensed work so he can feed himself and continue working on his own works.
His style is also more demanding than some other artists. Dude works on giant sheets of paper so he can cram all his tiny details
