I don't get it. Is it really that much healthier without the bun? Why not just have a regular burger and a salad?

Well since the bun was most likely going to be made from refined flour which is a simple carb that breaks down into sugar very quickly then yes it's healthier.

bread in itself is not bad. Very heavily processed bread is unhealthy, you should try to eat bread where the flour is as unprocessed as possible (i.e. definitely no white bread)

White bread, like Wonder bread or common burger buns are pretty bad. It's almost cake. Cheapie tier "whole wheat" bread is a little better, but not much. It's not usually truly whole grain flour. Real whole grain flour bread like from a bakery that has no preservatives (shelf life of under a week) is the best option healthwise. The fiber from the bran and germ is left in true whole grain flour which slows absorption, and won't spike blood sugar as much, but it also goes bad quicker. So industry doesn't like it because everything is so large-scale industrialized and long-distance shipping oriented. This is a great system for making money, but a really bad one for making great bread and keeping a healthy population.
