After opening and losing its vaccum, yes, it should be refrigerated.
Think about it this way, OP. If it's not packed in vinegar or salt, it has no resistance against the bacterial content that air introduced (and whatever utensil you stuck in there). Once opened, how long will it last? Give it the shelf life of whatever it would have been if it were fresh. For dairy, I'd go 2 days, since it was quite sterile to begin with, as a canned item, I'd keep it longer if it was mostly full, and less time if it was mostly empty, or if I had reheated it in the jar. More air. Week? Two weeks tops.
Anything in a jar other than jelly without that vinegar component, I absolutely toss within a month of opening, from chili garlic sauce to curry paste. This is why tube pesto or tube tomato paste is nice..no airspace.
