I honestly don't know how to get over it. How the fuck do people make friends? How do I stop feeling like a self conscious sad sack in pretty much any situation? Even with online people I get nervous and can't help but think they think I'm annoying and hate me.

Well going by your nerves from going to the grocery store/ talking to people online/ no irl friends and both craving and wanting to distance yourself from social interaction I must say that you have bad social interactions in your life hence the nerves and wanting distance as that is how you protect yourself but you know that will not make you happy, it will protect you but not make you happy.
Humans are very much like sheep and I'm guessing youve been alone for quite some time before you met your bf? and you watch alot of anime right? pretty much every single anime ever revolves around friendships so its to be expected that you too would want good friendships.
Your thoughts basically create your reality and what you surround yourself with. Losing empathy may have something to do with your life experiances with people.

Autism isnt an aversion to socialising, it's a difficulty with socialising. the former is usually introversion, the latter is more autism. as for the difficulty with empathising, a big bit about autism is the difficulty processing emotion the same way as others, so that's a big indicator.
Now for the "cure": you'll hate it initially but you NEED to go to places people go to socialise and try to make friends. Nowadays that's the go-to for autists who're sick of their own shit and want to try and change, so you might even bump into people who have the same issue(s) and you could make a friend that way. could tag along with your bf, which would definitly help.

Regardless, labels are irrelevant, doubly so without a diagnosis from a (non retarded) mental health professional. If you think you have empathy issues bordering on autism, then learn empathy (it's possible) and socialize to practice it, but please don't fish for special snowflake excuses on anime imageboards...
If I had to hazard a guess, the social anxiety comes from poor empathy -> hard to fit in -> hard to manage social expectations -> social performance anxiety -> avoidance. Anime and online are nice because you don't have to pickup retarded social cues to understand what's happening, but that turns into a trap when you become reluctant to step outside your comfort zone. Everyone is different obviously, so maybe I'm miles off.
