when charcoals are burning without smoke, do they release carbon monoxide? do i need the top part of this sort of grill to do the job?

Yes it releases CO2. I remember a family in England that died because they brought the BBQ inside the entrance to their large tent, even though it was pretty much out they still suffocated.
