I'm not saying that this is the exact cause, but I want to speculate something.
Before video games came around, we had the television and film industry being shaped. For about four decades during it's development, it was forced to follow the Motion Picture Production Code, or the Hays Code, where it could not necessarily delve into topics that might have seemed "shocking" back then. One of these rules was that the villain, at no point, was allowed to win or get away with their crime, so you couldn't really have a story about a villain's success then. The code was revoked before we reached the seventies, but the villain losing at the end was still one of the more popular types of stories being both written and viewed.
There's a fair chance a lot of people grew up enjoying the villain losing at the end of the day and wished to make their own stories of the villain also losing because that's how they were cultivated growing up. The trend still continues which is why most stories in any kind of media still revolve around stopping the villain and saving the day.
I do agree with you about games that try to utilize a morality system because they are flawed to where a player's decisions leave no room for debate. They make almost every action be black or white in extremes. As for games where you can play as the villain, I can't think of many examples off the top of my head either. The only one that comes to mind is Evil Geniuses and the original Dungeon Keeper (to an extent).
Sorry if that was a long read. It was something I've been thinking of myself. It's something that does need exploring.

Come to think of it, KotOR 2 actually did manage that pretty well. The plot of it is that the villain is this black hole in the force, and if left unchecked, he'll go around consuming everything until there's nothing left. You have to go and recruit the last few scattered jedi, who are all hiding from him. You can go for the pure light side route and be the typical good guy, and you can be a dark side character who just goes around killing them all instead of having them join you. But, there's actually a third option, where you can go full dark side but still recruit the others to your cause. The game actually makes it sort of difficult because you have to talk your way out of a few encounters, since the jedi you meet all realize how evil you are and assume that you're the one they're being forced to hide from.
It's neat because even though you're the bad guy, you're still fighting to take down somebody who's a threat to the force as a whole. That's a lot more depth than I would have normally expected out of a Star Wars game.

Before the whole "canon shutdown" of the expanded universe, Star Wars had a lot of interesting stories going for it. Not just the movies, but it explored other characters and scenarios like Jedi Academy and Republic Commando. Even some of the cartoons were really good. For KotOR, this was before Bioware started streamlining everything they did, so there were actual options versus choosing a color.
Going back to the lack of playable villains as a focus, there is one small game that I played a long while back where you don't realize that you are playing as the villain either until you get the true ending (there's a "neutral" ending) or you do specific sequences to trigger dialogue telling you that, but it's generally not possible on your first playthrough. It was a free horror game, so I don't know if that gives it away. It feels like if I give a title for it, it would spoil the game, but I don't mind giving it if you're curious. The only thing is that it doesn't really have a villainous approach to it, so you play it assuming one idea without really considering the alternate possibilities.

There are only a few. I have one for diapers and a pacifier one. There are also cloth diaper mods, but I haven't found them--all of my mods come from an old catalog I downloaded nearly half a decade ago which happened to be backed up on a hard drive. As such my mods are very outdated--the latest mod catalog appears to have a few more, but I don't have them.
My diaper mod is actually a bonus item from a gym bloomers mod. The texturing is...pretty poor, but they're actually pretty thick in the front, they're definitely not just recolored panties.
